    This paper determines the exact relationships that hold among the major Paleyā€“Wiener perturbation theorems for frame sequences. It is shown that major properties of a frame sequence such as excess, deficit, and rank remain invariant under Paleyā€“Wiener perturbations, but need not be preserved by compact perturbations. For localized frames, which are frames with additional structure, it is shown that the frame measure function is also preserved by Paleyā€“Wiener perturbations

    Systemic EBV+ T-cell lymphoma in elderly patients: comparison with children and young adult patients

    Fulminant Epsteinā€“Barr virus (EBV+) T-cell lymphoma in immunocompetent elderly patients is rare and its character has not been well defined. This study analyzed the clinicopathological features of five elderly patients (group A: 50ā€“84Ā years) and compared them with those of eight children and young adult patients with systemic T-cell lymphomas (group B: 10ā€“34Ā years). Group A more commonly presented with generalized lymphadenopathy (nā€‰=ā€‰3) than did group B (nā€‰=ā€‰1). Chronic active EBV infection (nā€‰=ā€‰3) and hydroa vacciniforme-like eruptions (nā€‰=ā€‰1) were seen in group B, while group A showed no evidence of chronic EBV infection, but did show chronic hepatitis B or C virus infections (nā€‰=ā€‰3). The histological and immunophenotypical findings were similar. All patients died within 1 to 14Ā months of diagnosis. These findings suggest that EBV+ T-cell lymphoma in elderly patients is a unique disease with an underlying derangement of T-cell immunity and failure to eradicate infected virus. Additional factors related to senility may play a role in the disruption of homeostasis between the virus and the hostā€™s immune system

    Clinical Characteristics and Risk Factors for Nosocomial Candidemia in Medical Intensive Care Units: Experience in a Single Hospital in Korea for 6.6 Years

    The aim of this study was to determine candidemia incidence among patients in a medical intensive-care unit (MICU) and the associated mortality rate and to identify risk factors associated with candidemia. We retrospectively performed a 1:3 matched case-control study of MICU patients with candidemia. Controls were matched for sex, age, and Acute Physiology and Chronic Health Evaluation (APACHE) II score. Candidemia incidence was 9.1 per 1,000 admissions. The most common pathogen was Candida albicans. Crude mortality was 96% among candidemia patients and 52% among controls (P<0.001). Mortality differed significantly between the groups according to Kaplan-Meier survival analysis (P=0.024). Multivariate analysis identified the following independent risk factors for candidemia: central venous catheterization (odds ratio [OR] = 3.2, 95% confidence interval [CI]=1.2-9.0), previous steroid therapy (OR=4.7, 95% CI=1.8-12.1), blood transfusion during the same admission period (OR=6.3, 95% CI=2.4-16.7), and hepatic failure upon MICU admission (OR=6.9, 95% CI=1.7-28.4). In conclusion, we identify an additional independent risk factor for candidemia, the presence of hepatic failure on MICU admission. Therefore, increased awareness of risk factors, including hepatic failure, is necessary for the management of candidemia

    A randomized, open-label study comparing low-dose clevudine plus adefovir combination therapy with clevudine monotherapy in naĆÆve chronic hepatitis B patients

    PURPOSE: Clevudine 30Ā mg showed potent antiviral activity with a marked post-treatment antiviral effect. However, long-term treatment with clevudine monotherapy induced resistance and myopathy in some cases. The objective of this study is to evaluate the preliminary efficacy and safety of the combination of clevudine 20Ā mg and adefovir compared to clevudine monotherapy. METHODS: Seventy-four patients were randomized to either a combination of clevudine 20Ā mg and adefovir or clevudine 20 or 30Ā mg and were treated for 2Ā years. The viral kinetics for 24Ā weeks, virological response [VR; hepatitis B virus (HBV) DNA less than 300 copies/ml], and the biochemical response [BR; normal alanine aminotransferase (ALT)] were assessed. RESULTS: There was no difference in baseline characteristics among the three groups. Viral kinetics study showed no statistically significant difference among them during 24Ā weeks. The combination group showed 95Ā % virological response with a statistically significant difference compared to the clevudine 30Ā mg (67Ā %) and 20Ā mg (71Ā %) groups (pĀ =Ā 0.0376). Biochemical response rates were similar in all groups (78ā€“94Ā %). No resistance was reported in the combination group, while 20Ā % of patients treated with clevudine 30Ā mg or 20Ā mg reported resistance during 2Ā years. Muscle-related symptoms such as myalgia (1 in clevudine 30Ā mg, 1 in the combination group) and muscle weakness (1 in clevudine 30Ā mg, 2 in clevudine 20Ā mg) were reported in five patients (7Ā %); of these, three patients discontinued the study. CONCLUSION: We concluded that the combination of clevudine 20Ā mg and adefovir produced a potent antiviral response together with a good resistance profile compared to clevudine monotherapy at 96Ā weeks in this pilot study

    The Prevalence and Evolution of Anemia Associated with Tuberculosis

    Tuberculosis (TB) may produce abnormalities in the peripheral blood, including anemia. However, the evolution of TB-associated anemia with short-term combination anti-TB chemotherapy has not been well elucidated. The aim of this study was to characterize TB-associated anemia by clarifying its prevalence, characteristics, and evolution, through involving large numbers of patients with TB. The medical records of adult patients with TB diagnosed between June 2000 and May 2001 were reviewed. Among 880 patients with TB, 281 (31.9%) had anemia on diagnosis of TB, however, the hemoglobin concentration was less than 10 g/dL in only 45 patients (5.0%). Anemia was more frequently associated with the female and old age. Good treatment response, young age (ā‰¤65 yr-old) and initial high hemoglobin were the predictive factor for resolution of anemia. In 202 patients with anemia (71.9%), anemia was normocytic and normochromic. During or after anti-TB treatment, anemia was resolved in 175 (64.6%) out of 271 patients without iron intake. The mean duration of resolution from the initiation of anti-TB treatment was 118.8Ā±113.2 days. In conclusion, anemia is a common hematological abnormality in patients with TB and close observation is sufficient for patients with TB-associated anemia, because TB-associated anemia is usually mild and resolves with anti-TB treatment

    A case study on swell correction of Chirp sub-bottom profiler (SBP) data using multi-beam echo sounder (MBES) data

    High-resolution marine seismic data acquisition and subsequent analyses are highly influenced by sea conditions, directly affecting data quality and interpretation. Traditional swell effect correction methods are effective in improving reflector continuity; however, they are less useful for enhancing travel time consistency at intersection points of crossing lines. To develop a robust swell-removal technique for a set of crossing lines multi-beam echo sounder (MBES) data and Chirp sub-bottom profiler (SBP) data were acquired. After generation of a time structure map of the sea-bottom converted from the final processed multi-beam data, a moving average was used to improve the event continuity of the sea-bottom reflection of the Chirp SBP data. Using the position of the Chirp SBP data, the difference between the travel time of the sea-bottom from the smoothed map and the original travel time of the sea-bottom is calculated as a static correction. The static correction method based on the MBES data was compared and verified using three different cases: (i) simple 2D swell effect correction on a line-by-line basis, (ii) comparing the swell corrections at the crossing positions of 2D lines acquired from different dates, and (iii) comparison of ties of intersection points between 2D lines after new swell correction applied. Although a simple 2D swell correction showed great enhancement of reflector continuity, only the full static correction using the newly proposed method using MBES data produced completely corrected reflection events especially at the crossing points of 2D lines

    Translation and validation of the Korean confusion assessment method for the intensive care unit

    <p>Abstract</p> <p>Background</p> <p>Delirium is a common problem and associated with poor outcomes in intensive care unit (ICU) patients. Diagnosis of delirium in ICU patients is limited and usually underdiagnosed by physicians. The Confusion Assessment Method for the Intensive Care Unit (CAM-ICU) is one of the most widely used screening methods for detection of ICU delirium. Our goal was to translate and validate the CAM-ICU for use in the Korean ICU setting.</p> <p>Methods</p> <p>Translation of the CAM-ICU was done according to the guidelines suggested by the Translation and Cultural Adaptation Group. For validation and interrater reliability assessment of the Korean CAM-ICU, two nurses independently assessed delirium in ICU patients and the results were compared with the reference evaluation, which was done by a psychiatrist using the Diagnostic and Statistical Manual of Mental Disorders IV (DSM-IV).</p> <p>Results</p> <p>Twenty-two patients were evaluated by two nurses and one psychiatrist expert independently. During the study period, we have continuously educated study nurses. Based on DSM-IV criteria, 16 out of 22 (72.7%) patients developed delirium. The sensitivities of the two nurses' evaluations using the Korean CAM-ICU were 89.80% for nurse 1 and 77.40% for nurse 2. Their specificities were 72.40% and 75.80% and their overall accuracy was 83.33% and 88.37% respectively. The Korean CAM-ICU was done with reasonable interrater reliability between nurse 1 and nurse 2 (Īŗ = 0.81, <it>p </it>< 0.001).</p> <p>Conclusions</p> <p>The Korean CAM-ICU showed good validity and could be incorporated into clinical practice in Korean ICUs.</p> <p>Trial registration</p> <p>ISRCTN: <a href="http://www.controlled-trials.com/ISRCTN50265663">ISRCTN50265663</a></p

    Flora of Vascular Plants in Ridgelines in the Palgongsa Procincial Park, Korea

    AbstractWe investigated and analyzed the flora of vascular plants around ridgelines in the Palgongsan Provincial Park to suggest fundamental data for establishing preservation plans of them. The flora of vascular plants around ridgelines was a total of 587 taxa including 102 familiies, 314 genera, 518 species, 58 varieties, 7 forma, and 4 subspecies. Rare plants designated by Korea Forest Service were 27 taxa including Semiquilegia mandshurica and Aristolochia manshuriensis. Endemic plants were 15 taxa including Clematis trichotoma and Carex okamotoi. Floristic special plants were a total of 100 taxa including all five classes. Naturalized plants were 13 taxa including Phytolacca Americana, ambrosia, and Taraxacum officinale
